Defeating Xesht, We That Are One, the pinnacle Breach boss, offers a chance to obtain the Hand of Wisdom and Action. Xesht resides within the Twisted Domain, accessible via the Realmgate on the Atlas using a Breachstone.

The Realmgate is near the Atlas's starting point. Use the home icon on the Atlas to locate it easily.

Alternatively, you can purchase HoWA on the Path of Exile 2 official trading website, though expect a high price, even for lower-tier versions.

Acquiring a Breachstone in PoE 2

Two methods exist for obtaining a Breachstone:

• Combine Breach Splinters: Farming Breach map nodes yields Breach Splinters. While relatively common, accumulating 300 requires extensive farming. Combine a stack of 300 to create a Breachstone.
• Currency Exchange: Purchase Breachstones (or Splinters) from Alva's in-game Currency Exchange. Expect to pay approximately 50-60 Exalted Orbs, though prices fluctuate.

No. Hand of Wisdom and Action is a unique drop from Xesht, not part of the regular loot pool (unlike Astramentis, Headhunter, or Polcirkeln). Therefore, using an Orb of Chance on White Furtive Wraps won't yield this item.
